To enroll, you need your Employee ID and your PIN.

PIN: Last 4 Digits of Employee ID and Full Birth Year; if your Employee ID is less than 4 digits, use your entire Employee ID

Examples:

Jane Smith, EID: 15, born 1/1/1980; PIN = 151980

John Doe, EID: 3412, born 1/1/1980; PIN = 34121980

Jane Doe, EID: 12345, born 1/1/1980; PIN = 23451980

Employees who wish to speak with a Benefits Counselor can do so by calling 1-877-286-2852 M-F, 7a – 5p PDT
