To review the plan, costs and to enroll, you will need your Employee ID and your Personal Identification Number (PIN). 

• Employee ID: Do not include any dashes or spaces. 
• PIN: Last 4 Digits of Employee ID and Full Birth Year; if your Employee ID is less than 4 digits, use your entire Employee ID (Note - if Employee Identification Number is fewer than 4 digits, PIN will be initially padded with 0s to fill out the four digits) 

Examples: 
Jane Smith, EID: 15, born 1/1/1980; PIN = 00151980 
John Doe, EID: 3412, born 1/1/1980; PIN = 034121980 
Jane Doe, EID: 12345, born 1/1/1980; PIN = 23451980
